Order of documents in a summary report

Good afternoon,
I know years back I came across a list of the minimum number of pages(forms/documents), and the order they are to appear, required for a complete appraisal report.

Can anyone please point me in the direction of this source.

I know we all use different pages in a report, some are client required forms, some are value added forms for marketing, but there must be a minimum required form list out there. I am interested in the minimum forms required and the order that they are supposed to appear in a report.
